By my child's voluntary participation in I.C.B.A., I hereby grant I.C.B.A., its agents, employees, coaches, volunteers, parents or any other person, licensee and other authorized media, including television, radio, the internet, I.C.B.A.'s website, social media, newspapers or any other publication, full and unrestricted permission to do the following:
1. Use and reuse the name, photograph, image, voice, likeness and biographical information of me or my child, including the use of such information or likeness on television or the Internet and in any other media for any purpose and for advertising and publicity in any media; or
2. Use, encode, digitize, copy, edit, extract, transmit and display on videotape, digital video stream or any other form of media, my child's participation in an I.C.B.A. basketball activity; or
3. Use and reuse my name, voice, photograph, image, biographical information or that of my child in connection with the I.C.B.A. website . This permission applies to all activities in which my child participates as part of I.C.B.A. including, but not limited to, all I.C.B.A. athletics, basketball, basketball camps, and any other related activities.
I understand that the above information will be distributed to members of the public and the media to: publicize I.C.B.A.'s activities as well as the performance of individual participants; promote I.C.B.A. as an organization in the sport community; and communicate information to participants and their families about the success of I.C.B.A.
I understand that the names and likenesses of the participants may appear on the I.C.B.A. website and that the performances may be broadcast on the Internet to interested I.C.B.A. individuals and members of the general public, and may be videotaped and/or digitally captured for subsequent broadcast, webcast and/or transmission.
This authorization includes, but is not limited to, perpetual rights to use internally and to license, sell or otherwise transfer the videotapes or digital files to third parties, and includes transmission and posting on the Internet. This authorization is irrevocable and royalty-free and I understand that I.C.B.A. will act in reliance on this authorization.
